Mankati Fullscale XT E3D hot end adapter

Original hotends were completely unsatisfactory so I changed to 2xE3D v5 hotends using this adapter plate.
Current plate works with one hotend.

Added a longer version of the Z-endstop because E3D hotend is longer.
