Phone fundus camera adapter

UPDATE March 2023: This adapter is not suited for phones where the light and camera are more than 10mm apart from each other. In order to work well one needs coaxial light source. I am planing on upgrading the adapter so it will also work with new phones.
Small gadget to make it easier to take fundus photos with a smartphone and a ophthalmic lens. This is a remix of oDocs design. The changes makes it easier to change lenses and adjust the distance between phone and lens. It also makes it possible to take macro pictures of externa by sliding the phone all the way up to the lens front.
Printed with PLA, 20% infill, 2 walls and 3 top/bottom layers.
In addition you need some screws and nuts:
14mm M4 cap head screw x 3 (x 2 for the phone holder part, and x 1 for fixing it to the rod).
10mm M4 cap head screw x as manny lens clips you need.
M4 nut for each screw.
M6 Hex bolt apx. 35-40mm long (30mm is too short) and a M6 nut.
