Summary

I decided to make my own minimalistic mount because I wanted the nozzle height exactly the same as the stock nozzle, and I wanted to retain the E3D fan shroud for proper cooling to all fins.
NOTE: This does require slight trimming of the extruder mounting plate, as well as the plastic piece that fits over the stepper motor and holds the lever.

Use at your own risk - I am not responsible for any injury or damage, etc.
Print with ABS at 100% infill. I had the best results printing the clamp with the curved side facing UP, and the mount with the curved side facing DOWN, with supports of course. You will have to drill out the through-holes a bit (intentional) and use shorter screws (available at almost any hobby shop).
Trim the aluminum mounting plate per the attached drawing. I used a Dremel with a tungsten carbide cutter and then finished with a sanding drum. Then file a notch into the plastic part that mounts to the extruder. You will see where you have to file when you go to test fit - just file where the top of the e3d would hit the plastic. I decided this was better than spacing the motor up higher than necessary.
Finally, cut the PTFE tube at an angle so it fits close to the hobbed bolt and won't let soft filament go anywhere but where it's supposed to.
